Release and Exchange of Information
In order for All Children Academics to work with your child’s outside professionals we must have a signed and completed “Release and Exchange.” Please turn in with your admissions application.

All Children that are taking medication whether administered or not at school, must fill out the medication form, and update as necessary.

Dietary Restriction Form
All Children that have food allergies, restrictions must fill out the dietary restriction form and update as necessary.

Current and new students must fill out a summer camp registration form, and return with a security deposit. New students to All Children Academics must fill out all admissions and State forms, but are not required to pay an application fee.
